I have a couple of MacBooks, 2011 and 2020 and in both cases the PSU adapter plug (UK spec) has a metal earth pin but is OC to earth. Using the extension lead instead of the adapter gives a high resistance path to earth (~1kΩ) but as Wonky and Hugh say, that's not adequate as an audio system earth.

I think it's well worth trying your old phone, especially if it has a headphone socket, with no sim card and all wireless connectivity switched off. We're not suggesting 'workarounds' (yet) but diagnostics/fault finding strategies. Until you can trace the fault to a particular device or cable you won't know if you have a dodgy phone, a dodgy mixer or a dodgy cable (or several). I'm guessing you mean 'the other input' not output? You just need a couple of phono to ¼" jack plug adapters but you'll still probably need to try the old phone as well using a headphone to 2 x phono plug cable.

The Duet 3 only appears to have 2 inputs according to the web page so there is no way to have three mics plugged in at once even with an external mic preamp so in your situation I'd go for an audio interface with more inputs.

Testing using a different source and cable and testing using differenputs is to try to isolate the problem, if it stops happening when you use the same phone and cable but with different inputs then it's likely the fault is with the phono input/channel, if it doesn't but using a different device and ...

If you can try a couple of phono to ¼" jack adapters and plug the phone into a different channel/pair of channels that should tell you where the fault is. Then try a different music source, a different device preferably with an analogue minijack output.
