I have an old Shure headset,

I can convert for wired use. I'm considering putting this transformer,
150/150 xformer:

In a belt-clip box, maybe swapping the mini xlr for a regular one.
However, I also have an old BSS ar133 DI box (with an XLR unbalanced input) that I suspect might serve the same purpose.
Any thoughts or suggestions before I break out the toolbox?

I think you could make life a lot easier for yourself by simply cutting off the mini four-pin connector and soldering on a male XLR3. Job done!
The dynamic capsule is inherently balanced, but because your specific model is intended for use with a budget radio pack they have wired it for an unbalanced output purely in that mini-plug. Hence all the warnings about phantom power and the shenanigans with a transformer.
None of that is required if you don't want/need the mini-plug connectivity. Just hack it off and solder on an XLR3m -- red wire to pin 2, black wire to pin 3, screen to pin 1.
